This workshop is open to all current practising general practice staff that are looking to improve Chronic Disease Management (CDM) service delivery in the Peel region.
Delivered locally, by national training organisation CDMPLUS, this face-to-face workshop will provide practical tips, networking opportunities and covers everything CDM including:
- Medicare, using MBS online, keeping up-to-date with changes, CDM item numbers and MBS billing combinations
- Care plans and health assessments
- Allied health pathways
- Home Medicines Review (HMR)
Eligibility
General Practitioners, Practice Managers, Nurse Practitioners, Nurses, Aboriginal Health Workers and Practitioners, Allied Health Practitioners, Medical Practice Assistants. Priority placement will be given to those practising in the Peel region.

This workshop is heavily subsidised with thanks to funding provided by the Australian Government Department of Health and Aged Care under the Primary Health Network Program. Standard external workshop fee usually $935pp inc GST.

Cancellation Policy
Applicants who give notice of cancellation on or before Sunday 15 October 2023 will receive a full refund of the course fee. Applicants who withdraw from the forum between Monday 16 October 2023 and Saturday 21 October 2023 will receive a 50% refund. Applicants who fail to attend the forum or who withdraw from the forum on or after Sunday 22 October 2023 will not receive a refund.
